Coordinate Home Health Care

Once Medicaid is approved, the second phase begins. In order to get the home aides needed evaluations need to be arranged, home health aides scheduled, a pooled trust may need to be established, and more.  I will coordinate all of the above for you to facilitate your care.

Advocacy Services

This service is aimed toward the adult children or loved ones of a nursing home resident who are unable to visit regularly to ensure proper care or attend case/care conferences.  I will visit weekly, inspect your loved one's condition and care as you would and attend the case/care conferences on your behalf to advocate for the resident.  You will receive a summary of the conference and consistent communications from me, either via phone or email as to their condition and well-being.

It is crucial that someone is regularly monitoring their care. Seniors residing in nursing homes receive better care when they have someone looking out for their best interest.
